Due to School Closures, we CANNOT PICK UP ANY BAND INSTRUMENT RENTALS

If you need to RETURN YOUR INSTRUMENT, you may do so in ONE the following ways:

• Ship the instrument (in a protective box) to our store address below:
• Pladd Dot Music - 38 North Main Street - Statesboro, GA 30458
• Fill out THIS FORM and we will pick it up when we are able to get back in school.

Please understand that you are responsible for the rental charges until we get the instrument back in our posession.
